Theory of Change

Long-term Outcome-based Objectives

The Oglala Lakota Division of Behavioral Health offers a variety of data-driven programs and services that have been specifically tailored to meet the needs of Oglala Sioux Tribal members. Through planning, implementation, and evaluation of high-quality programs and services, OL-DBH focuses on achieving five measurable long-term outcomes:


  1. Increase the proportion of individuals with substance use (SUD) and mental health disorders/serious mental illness (MHD/SMI) who receive treatment and are placed into an appropriate level of care annually. 
  2. Increase the proportion of youth (10 to 17 years old) with depression who receive treatment and are linked to treatment annually. 
  3. Increase alcohol, commercial tobacco, and drug use (ATOD) abstinence among youth (10 to 17 years old) residing on the Pine Ridge Reservation.
  4.  Reduce binge drinking behavior among adults (≥ 18 years) residing on the Pine Ridge Reservation. 
  5. Reduce past 30-day, 12-month, and lifetime use of substance use of youth and adults. 

All program activities and services are guided by these five outcome objectives and long-term progress within these priorities is carefully assessed at the population level as well as among those clients seeking services. To make progress toward the long-term outcome objectives OL-DBH has established the following output goals designed to systematically improve the program's services and reach over time:

Goal 1: Health Surveillance and Monitoring

Assess the need for behavioral health services within the Pine Ridge Reservation.

1.1 Monitor the prevalence of substance use and mental health disorders on the Pine Ridge Reservation. 

1.2 Assess existing gaps in prevention, treatment, and recovery support services. 

1.3 Assess the quality of existing services through program evaluation as part of continuous quality improvement. 

1.4 Collaborate with the OST’s epidemiology/public health stakeholders to identify specific tribal needs related to behavioral health. 

Goal 2: Access to Quality Care

Increase the number of individuals who are provided behavioral health services on the Pine Ridge Reservation.   

2.1 Increase awareness of tribally based prevention, treatment, and recovery support resources among residents of the Pine Ridge Reservation and behavioral health professionals throughout western South Dakota. 

2.2 Increase the number of clients placed into residential/inpatient treatment. 

2.3 Increase the number of clients participating in outpatient treatment. 

2.4 Increase the number of clients participating in medication for substance use disorder treatment. 

2.5 Increase the number of clients participating in transitional living and/or aftercare services following a formal treatment service.   

Goal 3: Tribal Behavioral Health Capacity

Increase the capacity of the Oglala Sioux Tribe's behavioral health workforce to plan, implement, and evaluate best and promising practices demonstrated to prevent and treat substance abuse and mental health disorders. 

3.1 Facilitate training and technical assistance related to mental health, addiction, and family counseling competencies as defined by SAMHSA. 

3.2 Facilitate training and technical assistance related to behavioral health science, counseling techniques, and evidence-based practices. 

3.3 Increase behavioral health professionals' ability to collect, interpret, and utilize program evaluation data. 

Goal 4: Direct Client and Community Services

Assess the need for behavioral health services within the Pine Ridge Reservation.

4.1 Implementation of screening and diagnostic services (addiction & mental health). 

4.2 Implementation of Targeted Case Management.

4.3 Implementation of behavioral health interventions for youth and adults (I.E. addiction treatment, MRT, recovery support, diversion services). 

4.4 Implementation of skills-based transitional living services within the outpatient and residential settings.

4.5 Collaboration with tribal-based partners to coordinate the implementation of detoxification, crisis care, and other in-patient services. 

Goal 5: Systems and Environmental Change

Promote systems and environmental change that cultivate well-being and sober living. 

5.1 Implement community prevention activities in congruence with the OST Strategic Prevention Plan. 

5.2 Plan and deliver health communication to inform the public of the health, economic, and social consequences of ATODs and local behavioral health resources. 

5.3 Develop policy change resources that utilize local epidemiological data. 

5.4 Implement community-based activities that provide prevention education within the context of Lakota culture.
